Abstract

Dramatically up-regulated in the dorsal horn of the mammalian spinal cord following inflammation or nerve injury, neuropeptide Y (NPY) is poised to regulate the transmission of sensory signals. We found that doxycycline-induced conditional in vivo (Npytet/tet) knockdown of NPY produced rapid, reversible, and repeatable increases in the intensity and duration of tactile and thermal hypersensitivity.

Remarkably, when allowed to resolve for several weeks, behavioral hypersensitivity could be dramatically reinstated with NPY knockdown or intrathecal administration of Y1 or Y2 receptor antagonists. In addition, Y2 antagonism increased dorsal horn expression of Fos and phosphorylated form of extracellular signal-related kinase.

Taken together, these data establish spinal NPY receptor systems as an endogenous braking mechanism that exerts a tonic, long-lasting, broad-spectrum inhibitory control of spinal nociceptive transmission, thus impeding the transition from acute to chronic pain. NPY and its receptors appear to be part of a mechanism whereby mammals naturally recover from the hyperalgesia associated with inflammation or nerve injury.

67 percent of Dr. Baraniuk’s chronic migraine patients have CFS

Dr. James Baraniuk, Assistant Professor of Medicine, Georgetown University

Accompanying Document: CFS Research: Transition from the Qualitative Consensus Tradition to a Quantitative, Translational, Hypothesis-Driven, Systems Biology Strategy

Dr. Baraniuk opened his presentation by saying that he was moved to be giving his presentation in the John Eisenberg Memorial Lecture Room. Dr. Eisenberg was the department chairman at Georgetown University and the professor who gave Dr. Baraniuk his start in clinical research.

One of the things that Dr. Eisenberg encouraged was thinking out of the box. Those in CFS have to make a change from the qualitative approach and consensus tradition toward a hypothesis-based type of investigation and new methods, said Dr. Baraniuk. The bywords now are using a “systems biology” or overall approach as to what is going on in multiple areas within an individual.

He gave the example of a patient evaluation that compares pain symptoms (headache, myalgia, arthralgia, sore throat, tender lymph nodes) and psychoneural symptoms (cognitive memory problems, sleep disturbances, exertional exhaustion) with fatigue.

The hypothesis: The sum for the minor criteria will be proportional to the level of fatigue. Significant fatigue for the diagnosis was defined as moderate (3) or severe (4).

Taking the sum of the minor criteria and comparing it with significant fatigue split the population into normal subjects, those with chronic idiopathic fatigue (CIF), and those with CFS. There was no difference by age or gender. Using the sum of minor criteria was superior to using SF-36 to separate the controls, those with CIS, and those with CFS. The implications for quantification of CFS criterion severities:

Scoring the severity of each CFS criterion provides more information about each individual.

The sum of the minor criteria correlates with fatigue and is a good predictor of CFS.

The next step is to explore whether cluster or multivariate analysis will identify subsets of CFS subjects in order to find out if there are common mechanisms that are creating subsets of CFS patients. Dr. Baraniuk said that he wants to use the current grant to find objective markers that will go along with those potential subsets. The grant (RO1 ES-015382) is funded by the National Institute of Environmental Health Sciences (NIEHS).

Researchers are:

Doing lumbar punctures in 50 CFS and 50 control subjects to analyze the proteomics of the cerebrospinal fluid.

Making psychometric comparisons using a lengthy questionnaire

Making pain comparisons using dolorimetry, isometric hand grip studies, and capsaicin skin tests for neurogenic inflammation.

A problem encountered during the study was spinal headaches, which are very severe and occur after a lumbar puncture. With the idea of making lemons out of lemonade, researchers decided to see what could be learn from spinal headaches about CFS pathology. Researchers discovered that 88 percent of CFS subjects had migraines. In checking the literature, researchers found that 84 percent of the migraine populations report fatigue. In fact, 67 percent of Dr. Baraniuk’s chronic migraine patients have CFS. This statistic suggests that there is more than a chance relationship and that perhaps there is a mechanism that is shared.

Migraines plus CFS suggests:

Autonomic abnormalities

Neurogenic inflammation

very likely contribute to the high level of overlap between migraines and CFS. From a treatment perspective, if you identify migraines in 88 percent of your CFS people, you should get them started on appropriate therapy.

From this we wanted to look more at the autonomic nervous system.

The hypothesis: Autonomic control during isometric hand grip maneuvers will be dysfunctional in CFS but not in healthy controls.

Subjects squeezed a hand strain gauge as hard as they could.

Researchers measured blood pressure, heart rate, and nasal dimensions (surrogates of sympathetic nervous system vasoconstriction) during a restful “SHAM” contraction.

Subjects squeezed a hand strain gauge at 30 percent of maximum for as long as they could; researchers determined autonomic responses.

There is something called the muscle reflex. When muscles are activated, nerves stimulate the brain stem and the brain stem stimulates the sympathetic nervous system centers, which send out messages that work to increase blood flow to the local contracting muscle. The dynamics of the squeeze and release of the hand grip stimulate multimodal Type C neurons. We used to think that these neurons would have just one sensory receptor and would mediate just one sensation.

Now it’s apparent that they have a wide variety of receptors, especially the multi-modal neurons. It turns out that the acid that’s released in the muscle will activate the acid ion channel that then leads to great up regulation of the sensitivity of the TRPV, which depolarizes the nerves and sends an incredibly strong pain message to the brain. In turn, that leads to sympathetic discharge of a variety of chemicals, one of which is ATP, which if it’s released in the local muscle area, will have a positive feedback effect increasing the pain.

When you stimulate the sympathetic nervous system, you actually squeeze down on the blood vessels in your nose and as a result, you have an increase in nasal patency. This turns out to be a good measure of autonomic function or in the case of CFS, dysfunction.

Isometric Hand Grip in CFS

Controls had slightly higher maximum muscle strength, equivalent duration of muscle contractions, and similar pain scores. Researchers expected that the CFS people would have higher pain scores.

As far as the autonomic component was concerned, the first factor that changes during a contraction is that the diastolic blood pressure begins to increase. That’s also the measure that goes up the highest in the healthy controls. There’s a very significant difference between the controls and the CFS people.

The systolic blood pressure goes up next, but usually very late in the contraction, and similarly, the heart rate goes up fairly late.

As far as the nose goes, the cross-sectional area for air flow also goes up in the controls significantly greater than in the CFS people. In CFS there’s a group that has a swelling of their mucosa when they should be constricting.

Conclusion: Mucosal and systemic sympathetic reflexes are dysfunctional in the pathogenesis of CFS and the nonallergic rhinitis of CFS. The primary defect is apparently autonomic dysregulation in the brain stem.

"It’s all in your head.” That’s now the mission statement for our laboratory,” said Dr. Baraniuk. “It’s all in your head because that’s where most of your nerves are. And that’s where the problem lies, I believe.”

“That led to our approach to look at cerebral spinal fluid. By way of review, you have blood flowing into a central structure in the center of the brain. As that blood flows through that structure, you have a very carefully generated fluid produced—the cerebral spinal fluid. It surrounds the brain, circulates throughout the day, goes down your spinal column, and we can collect it by doing a lumbar puncture.”

Hypothesis: Central nervous system dysfunction is a critical component of CFS and related syndromes such as FM, Persian Gulf War Illness, multiple chemical sensitivity, irritable bowel and bladder syndromes, and other allied conditions. Analysis of the fluid flowing from the brain will identify factors that may indicate the pathology of CFS to direct the creation of new treatments, and serve as diagnostic biomarkers for future testing.

Researchers obtained cerebrospinal fluid from Gulf War Illness (GWI) subjects (most also had CFS), FM subjects (many had CFS), and control subjects for a total of 62 subjects. Researchers compared pooled samples from GWI, pooled samples from CFS, and pooled samples from controls. The identified the proteins that were only present in the GWI and CFS people. They also took individual specimens from the CFS and GWI people and compared them to individual specimens from control subjects.

Dr. Baraniuk: It was mind boggling because we ended up with about 10 proteins that were present only in our CFS group and not present in the control group. The odds of this are about 10 to the minus 15. We’re currently doing the proteomic analysis using a high-end orbitrap mass spectrometer to see if we get the same pattern. The key proteins from the CFS-related proteome, I think, give us some potential clues as to what’s going on (chart from presentation slide):

With the new spectrometer, researchers get about 100 times more ions per run, 100 times more peptides, and about 20 times more proteins per run. We haven’t broken the code on the patient diagnosis in order to stay unbiased, so we don’t know what the mix of controls versus CFS is in the first two-dozen samples. A total of 4,075 were identified in the 24 samples.

The orbitrap system has really dropped down the level of sensitivity so that we’re picking up a lot of proteins that we could not pick up previously. We have over 200 unique proteins from these 24 samples. We had a large number of adhesion markers and it will take awhile to sort through the many different names that appear when you start doing the bioinformatic searches. After the next batch, we’ll probably be able to start doing quantitative analysis to compare the abundance of proteins between our CFS and control group.

We’re making progress—we’re getting there. Although this started out with some people thinking that it would be just a blind hog searching for acorns, I felt that we would find specific illness-related proteins. Now, based upon the distribution the types of proteins that we’ve found, we’ve been able to extend our work into other grants in other areas where we’re currently funded and are starting, for instance, a clinical treatment trial that I hope to adapt to CFS.

Committee Discussion

Dr. Baraniuk: It’s important for all of these that we identify by mass spectrometry to go on and do a quantitative assay. For all the proteins, we have that work to do once we identify which ones are significantly different. Specifically for the cytokines, we’re comparing plasma and cerebral spinal fluid.

Dr. Klimas: Are you looking for any pathogens?

Dr. Baraniuk: In the previous study we searched for all bacteria and all viruses and found one protein. But is was not a viral protein.

Resveratrol

In a separate body of research, scientists determined that a 10 mg daily dose of resveratrol was associated with lowered insulin resistance in Type-II diabetics.

Resveratrol is a powerful protective anti-fungal compound found in the skin of red grapes. Known to activate a longevity signaling gene (SIRT-1), resveratrol has been shown to extend lifespan by lowering the risks associated with systemic inflammation.
The polyphenol may help to prevent cancer development, cardiovascular disease and dementia, and it is now shown to impede the development of diabetes by regulating insulin sensitivity.

Dag 422 Kamperen

Dag 419 Roze garnalen

We planten rustig voort.

"Wil u dat even opschrijven ajb?"

"Excuseer?"

A few years ago, we found that people are much happier with irreversible than reversible decisions because we rationalise the former but not the latter. Someone mentioned this was the essential difference between living with or marrying your girlfriend. If your wife does something mildly annoying you shrug your shoulders, but when your girlfriend does it you wonder if you should keep shopping. As soon as I saw that result I went home and proposed.

Dag 417 Te mager

Mijn BMI is nu 17.

Dr. Brownstein is one of the top experts on iodine. About radiation exposure, he explains: "If there is enough inorganic, non-radioactive iodine in our bodies, the radioactive fallout has nowhere to bind in our bodies. It will pass through us, leaving our bodies unharmed.

The U.S. RDA of iodine is 150 micrograms (mcg) for adults, while 220 mcg and 290 mcg are recommended for pregnant and lactating women, respectively. These quantities were established to effectively prevent goiter but do not provide for the body's other needs for optimal thyroid, endocrine or immune system function, nor are they sufficient dosages for the prevention of cancer.

Dag 416 Tuinfrees

Dag 414 Black out

Het was een zwart gat, one of the many.

Dag 413 NIH workshops

Menig briljante ingeving is erdoor opgeslokt.

Het is eigen aan een virale aandoening, it comes and it goes. Dus het is even weg.

Delegeren gaat me goed af.

Vaccine Researcher Indicted for Fraud

According to CNBC, “Paul Thorsen, the principal coordinator of multiple studies funded by the CDC used to deny a vaccine/autism link was indicted April 13th on thirteen counts of fraud and nine counts of money-laundering. The charges relate to funding for work he conducted for the CDC which claimed to disprove associations between the mercury-based vaccine preservative, thimerosal, and increased rates of autism.” This was first reported by the consumer group, SafeMinds.

I remember when these studies came out. They came out in rapid-fire succession just after previously-released studies began to show there was a link between autism and mercury-preserved vaccines. I was not surprised that the Thorsen articles came out so fast as Big Pharma had to do something to quiet the growing complaints about mercury in vaccines.

Do I think that mercury-containing vaccines cause autism? Before I answer that, let me state a few points. I believe there is no justification for ever injecting in any living being the third most toxic element known to mankind. As the wise man Forrest Gump said, “Stupid is as stupid does.” What do you expect will happen when you inject a known neurotoxin, in large amounts, to a small baby?

I have no doubt that mercury toxicity, in large part from vaccines, is probably one factor responsible for the rapid rise of autism. Even if mercury is not one of the underlying causes of autism, common sense as well as biochemical sense would dictate that mercury exposure is not a good idea. In fact, mercury toxicity is a common problem in many of the patients that I see. However, in our toxic world, there are many other items we are exposed to on a daily basis that can also disrupt the normal neurological development in a newborn such as lead, arsenic, nickel, cadmium and bromine.

What can you do? Number one; don’t let anyone inject mercury in your body. Most of the flu vaccines still have mercury in them; they should be avoided. Next, don’t let any dentist put a mercury filling in your mouth. Any dentist that still defends the use of mercury does not understand biochemistry and does not deserve your business. Next, eat a healthy diet free of pesticides and other chemicals that are harmful to your body. Finally, take the appropriate supplements to ensure that your detoxification pathways are optimally functioning.

Which supplements should you take? There are many items that aid the detoxification pathways including vitamins C and E, alpha lipoic acid, magnesium and glutathione.

Waarom vitamine D ontsteking remt

Vitamin D: the alternative hypothesis.

Autoimmun Rev. 2009 Jul;8(8):639-44. Epub 2009 Feb 12.

Abstract

Early studies on vitamin D showed promise that various forms of the "vitamin" may be protective against chronic disease, yet systematic reviews and longer-term studies have failed to confirm these findings. A number of studies have suggested that patients with autoimmune diagnoses are deficient in 25-hydroxyvitamin D (25-D) and that consuming greater quantities of vitamin D, which further elevates 25 D levels, alleviates autoimmune disease symptoms.

Some years ago, molecular biology identified 25 D as a secosteroid. Secosteroids would typically be expected to depress inflammation, which is in line with the reports of symptomatic improvement. The simplistic first-order mass-action model used to guide the early vitamin studies is now giving way to a more complex description of action. When active, the Vitamin D nuclear receptor (VDR) affects transcription of at least 913 genes and impacts processes ranging from calcium metabolism to expression of key antimicrobial peptides. Additionally, recent research on the Human Microbiome shows that bacteria are far more pervasive than previously thought, increasing the possibility that autoimmune disease is bacterial in origin. Emerging molecular evidence suggests that symptomatic improvements among those administered vitamin D is the result of 25-D's ability to temper bacterial-induced inflammation by slowing VDR activity. While this results in short-term palliation, persistent pathogens that may influence disease progression, proliferate over the long-term.

Jodium in druppelvorm heeft ongekende toepassingsgebieden

Iodine is by far the best antibiotic, antiviral and antiseptic of all time - Dr. David Derry

Dr. Derry says that iodine is effective "for standard pathogens such as Staphylococcus, but also iodine has the broadest range of action, fewest side effects and no development of bacterial resistance." There is a world of difference between using an antibiotic – anti-life substance – and an antibiotic, antiviral and antifungal substance like iodine, which is life serving because it is a basic and most necessary nutritional substance.

Iodine kills single celled organisms by combining with the amino acids tyrosine or histidine when they are exposed to the extra-cellular environment. All single cells showing tyrosine on their outer cell membranes are killed instantly by a simple chemical reaction with iodine that denatures proteins. Nature and evolution have given us an important mechanism to control pathogenic life forms and we should use it and trust it to protect us in ways that antibiotics can't.

The way to combat antibiotic resistance is not bigger, better, stronger antibiotics but, rather, no antibiotics at all. Instead, other molecular weapons are available with the ability to disable bad germs without bothering good ones. Iodine is the ideal broad spectrum antibiotic that is not an antibiotic - it is not against life. Not against human life that is but you can hear the little pathogens screaming as high enough levels of iodine fan out through the system. Meaning all the viruses, bacteria, yeasts and molds that are threatening us are threatened with instant death when iodine is used orally to fight infection.

Mutating viruses

Did you know that a nutritional deficiency can cause a virus to mutate to a more virulent form? That is the news from the United States Department of Agriculture (USDA) who are reporting that a human virus, normally harmless in laboratory mice, mutated into a heart-damaging pathogen when the animals were raised on a diet devoid of the essential element selenium. And, once mutated, the virus continued to damage hearts - even in mice that got ample selenium in their feed.

The importance of this is not limited to nutritionally-deprived populations, say researchers with the University of North Carolina and Agricultural Research Service of the government, who collaborated on the studies. In theory, one selenium-deficient person or animal could produce a new family of virus mutants that could cross species and spread worldwide, causing disease even in well nourished people.

The USDA is now officially on record that nutritional deficiencies cause viral mutations and they expect to find the same results with vitamin-E-deficient mice because both selenium and vitamin E are nutrients that serve as antioxidants in the body. This means that the government is recognizing that free radicals and oxidative stress affects the world of pathogens creating super bugs out of regular critters. They are even going as far as saying that this may help explain the many new strains of influenza virus arising in China, which has widespread selenium-deficient areas.

Virology of the post-polio syndrome

Andreina Baj‌, Salvatore Monaco‌, Gianluigi Zanusso‌, Elisa Dall’ora‌, Laura Bertolasi‌ & Antonio Toniolo‌†† Author for correspondence

The three poliovirus serotypes (PVs) cause acute paralytic poliomyelitis. Decades after being hit by polio, survivors may develop a condition known as post-polio syndrome (PPS).

PPS is characterized by extreme fatigue, progressing muscular weakness and chronic pain. The pathogenesis is unclear and, thus, empirical therapies are employed.

PVs are known to be able to persist in infected host cells both in vitro and in vivo. The understanding of PV genomes has made it possible to set up sensitive and specific molecular tests capable of detecting minute amounts of virus in samples from PPS patients.

Current data indicate that complete PV genomes (or genomic fragments) remain present, decades after acute paralysis, in the CNS of these patients. Virus persistence is hypothesized to bring about chronic inflammation, immune-mediated injury and decreased expression of neurotrophic factors.

Establishing a pathogenetic link between PV persistence and PPS would be extremely relevant to the development of an etiologic therapy aimed at virus eradication.

There is no question that radioactive particles will be carried by the jet stream and deposited over North America. In fact, there have been multiple reports of radiation spikes occurring in a number of states. Now, the amount of radiation reported has been low. But, do not let that fool you. If you are iodine deficient, your body will try to absorb iodine from whatever source it can get it from, radioactive particles from Japan included. ...

As I mentioned in a previous post, it is important to ensure that you are iodine sufficient. How can you do that? The first step is to work with a health care provider who is knowledgeable about iodine. Next, you can get your iodine levels checked. Many labs will do this with a simple urine test. FFP labs and Hakala Labs do the most complete test on iodine, the 24 hour loading test. ...

Finally, ensure that you are taking adequate amounts of inorganic, non-radioactive iodine in order to maintain whole body iodine sufficiency. Iodoral (Optimox), Iodozyme HP (Biotics) and Lugol’s solution are examples of this type of iodine that are effective for maintaining whole body iodine sufficiency. The amount of iodine needed to achieve whole body sufficiency, for most of my patients, varies between 6-50mg/day.

The best results with iodine therapy are when it is combined with a holistic treatment regimen. This includes taking vitamin C and salt. Furthermore, correcting nutrient imbalances and detoxifying will help. Finally, ensure that you stay hydrated. ...

There is no need to panic over the Japanese disaster. You can use this situation as a wake-up call to ensure that you are doing all that is possible for maintaining your optimal health. Finally, remember to educate yourself about iodine so that you can make the best health care decisions.
